Billing

CHC will file all Medicare and Medicaid Claims.
CHC will file most private insurance claims.
Patients are responsible for paying deductibles and co-payment at the time of the visit.
Patients eligible for an income-based, sliding-fee discount will be asked to apply for these services in advance and present proof or statement of income, or sign a statement declaring their status.
Patients eligible for sliding-fee discounts are expected to pay their copayment at the time of service.
Patients are expected to pay in full at the time of visit when CHC is not filing insurance. You will receive a receipt so that you may file your own insurance.
Patients with a previous unpaid balance must pay for that day’s visit and will be encouraged to set up a payment plan for any previous balance.
